Job Description
Licensed Practical Nurse (LPN) - Medical & Behavioral Health Clinic Flourish Medical and Behavioral Health Group is seeking a dependable, compassionate, and motivated Licensed Practical Nurse (LPN) to join our growing medical and behavioral health practice. This is a part-time position, approximately 32 hours per week , designed for an LPN who enjoys working in a collaborative outpatient environment and wants to be actively involved in patient care, clinical support, care coordination, and the day-to-day operations of a growing practice. Our clinic provides both medical and mental health services , along with Chronic Care Management (CCM) and Transcranial Magnetic Stimulation (TMS) services. The ideal candidate will be comfortable wearing multiple hats and working closely with the provider, clinical staff, front-office staff, and patients. Position Summary The LPN will provide direct clinical support to patients and the healthcare team while also assisting with administrative and care-coordination responsibilities. This position is ideal for an LPN who is organized, dependable, comfortable communicating with patients, and able to transition between clinical and administrative responsibilities throughout the day. The successful candidate will help ensure that patients receive timely, compassionate care while helping the clinic operate efficiently. Key ResponsibilitiesPatient Care & Clinical Support Perform patient triage by phone and in person. Assess patient concerns and determine appropriate next steps within the LPN's scope of practice. Communicate patient concerns and clinical information to the provider. Obtain and document vital signs and other appropriate clinical information. Assist with patient intake and rooming. Administer medications and injections as ordered and within scope of practice. Perform venipuncture and draw blood specimens. Process and prepare laboratory specimens according to office procedures. Assist with laboratory coordination, results, and follow-up. Provide patient education as directed by the provider. Assist with medication-related questions and clinical follow-up. Assist providers with examinations and procedures as needed. Monitor patients before, during, and after appropriate treatments or procedures. Maintain accurate and timely clinical documentation in the electronic medical record. Follow all clinic policies, infection-control procedures, HIPAA requirements, and applicable state and federal regulations. Triage & Patient Communication Respond professionally and compassionately to patient calls and clinical questions. Triage symptoms and concerns according to established protocols. Recognize urgent or potentially emergent situations and appropriately escalate them to the provider or emergency services when necessary. Communicate test results and provider recommendations to patients as directed. Assist with patient follow-up and care coordination. Maintain professional communication with patients, pharmacies, laboratories, specialists, and other healthcare organizations. Prior Authorizations & Insurance Support Assist with medication and treatment prior authorizations. Gather necessary clinical documentation, medication history, diagnoses, and supporting information. Submit and track prior authorization requests through insurance portals and other appropriate systems. Communicate with insurance companies, pharmacies, and specialty programs as needed. Follow up on pending, approved, denied, or appealed authorizations. Assist the provider and clinical team with obtaining documentation needed to support medical necessity. Maintain organized records of authorization requests and outcomes. Chronic Care Management (CCM) The LPN will assist with the clinic's Chronic Care Management program , including: Supporting patients with chronic medical conditions. Assisting with care coordination and patient follow-up. Reviewing patient needs and communicating concerns to the provider. Helping patients understand their care plans. Assisting with medication reconciliation and adherence. Contacting patients for scheduled CCM activities. Documenting care-management activities appropriately in the electronic medical record. Assisting with coordination between the patient, provider, pharmacy, specialists, and other healthcare resources. Helping identify patients who may benefit from additional care-management services. TMS Services The LPN will also assist the clinical team with Transcranial Magnetic Stimulation (TMS) services.
