Job Description

Liberty Health is seeking a compassionate Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you'll deliver direct patient care, administer medications, monitor vital signs, perform basic treatments, and accurately document in electronic health records. The LPN supports RNs and providers in developing and following care plans, coordinates with interdisciplinary teams, educates patients and families, and promotes safety and infection control. Responsibilities Provide direct bedside and outpatient nursing care under RN/physician supervision Administer medications and injections per orders and scope of practice Monitor, record, and report vital signs and patient status changes Perform basic treatments, wound care, and specimen collection Document care accurately in electronic health records (EHR) Assist with development and implementation of patient care plans Educate patients and families on treatments, medications, and self-care Maintain infection control, safety, and quality standards Collaborate with interdisciplinary team to coordinate services and follow-up Support case management and care coordination activities as assigned Required Skills Vital signs monitoring Medication administration Wound care and basic treatments IV therapy (per state scope) Electronic Health Records (EHR) documentation Patient and family education Infection prevention and control Care planning support Basic life support (BLS) skills Care coordination and case management support
