Overview We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long Term Care team supporting individuals with developmental disabilities. The ideal candidate will possess a strong background in patient care within long-term care settings, demonstrating compassion, professionalism, and a commitment to delivering high-quality nursing services. This role offers an opportunity to make a meaningful difference in the lives of residents while working within a dynamic healthcare environment. Responsibilities Provide direct patient care to residents in accordance with established nursing protocols and care plans. Administer medications, including injections and IV infusions, following proper procedures and safety guidelines. Monitor vital signs, conduct physical examinations, and assess patient conditions regularly to detect changes or concerns. Document all patient interactions accurately using electronic health record (EHR) systems. Collaborate with interdisciplinary teams to develop and update individualized care plans tailored to each resident's needs. Participate in clinical team admission decision making, ensuring our agency can appropraitely support an individual within our treatment modality Monitor the e-MAR system to ensure accurate medication administration, including collaborating with operations department if deficiencies are noted Training direct-care staff on appropriate personal care techniques, including changing adult briefs, tooth brushing, nail clipping, etc. Participate in discharge planning and coordinate with families and healthcare providers for seamless transitions of care. Skills Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Experience working in long-term care facilities or skilled nursing facilities preferred. Knowledge of long-term care regulations, dementia care, Alzheimer's care, and memory support strategies. Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation. Strong clinical assessment skills with experience in patient monitoring and reporting Familiarity with IV infusion techniques, medication administration, and infection control practices. Ability to communicate effectively with residents, families, and multidisciplinary teams while demonstrating compassion and professionalism.
