We're seeking a compassionate and skilled Home Care Coordinator (LPN) to join our interdisciplinary care team. Position Summary The Home Care Coordinator (LPN) plays a vital role in coordinating, implementing, and monitoring home-based services for participants. This position ensures delivery of high-quality, person-centered care aligned with individualized care plans and supports participants across all care settings. You'll serve as a key liaison between participants, caregivers, and the interdisciplinary team to ensure seamless, compassionate care every step of the way. Key Responsibilities Coordinate and oversee all home care services in alignment with participant care plans. Conduct in-home visits and assist with participant assessments and reassessments. Collaborate with the interdisciplinary team (IDT) to support clinical, functional, and psychosocial needs. Supervise Home Health Aides and Personal Care Attendants to ensure quality care delivery. Coordinate transitions of care, including hospital and facility discharges. Arrange and manage Durable Medical Equipment (DME) and home care supplies. Monitor medication adherence and identify health status changes. Educate participants and families on care plans and health management. Maintain accurate documentation within electronic health records. Qualifications Current North Carolina LPN license (required). Graduate of an accredited school of nursing. CPR certification (required). Minimum 1 year of nursing experience ; home health or community-based care preferred. Experience working with the geriatric population strongly preferred. Valid driver's license, reliable transportation, and insurance. Strong communication, organizational, and teamwork skills. Why Join Elderhaus?
