3x12 Days | 3x12 Nights (Both shifts available)

Direct Hire Pay Range:
$60,000 - $70,000 yearly (plus shift differentials)
Sign-On Bonus:
Up to $12,000 (paid incrementally)

Senior residents (average census 46-54 residents) Position Summary The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Skilled Nursing and Long-Term Care setting. The LPN is responsible for administering medications and treatments, monitoring resident health, updating care plans, and ensuring residents receive safe, compassionate, and high-quality care in accordance with established nursing standards and individualized care plans. Key Responsibilities Administer medications and treatments according to physician orders and nursing standards. Monitor residents' physical and emotional conditions and report changes to the healthcare team. Assist in developing, implementing, and updating individualized resident care plans. Maintain accurate and timely documentation of resident care and treatment. Ensure compliance with facility policies, procedures, and regulatory requirements. Provide compassionate care while promoting resident dignity, comfort, and safety. Communicate effectively with residents, families, physicians, and interdisciplinary team members. Supervise and support nursing assistants and other assigned staff members. Assist with resident assessments and coordinate appropriate interventions. Participate in quality improvement and resident safety initiatives. Required Qualifications Graduate of an accredited nursing program. Active LPN License in the state of practice. Current BLS (Basic Life Support) certification. Minimum of 3 months of Long-Term Care (LTC) experience as an LPN. Strong knowledge of nursing procedures, medication administration, and resident care practices. Ability to communicate effectively in English, both verbally and in writing.
