6a-6p/6p-6a with rotating weekends Job Summary Emerald Nursing & Rehab Legacy Pointe, a leading provider of exceptional healthcare services, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team. As an LPN, you will play a vital role in delivering high-quality patient care and promoting a positive patient experience. Responsibilities:

Provide direct patient care to our residents, including administering medications, performing treatments, and assisting with daily living activities Develop and implement individualized care plans, in collaboration with our interdisciplinary team Monitor and report patient progress, identifying and addressing any changes or concerns Collaborate with other healthcare professionals to ensure seamless and effective patient care Maintain accurate and detailed records of patient care and treatment plans Participate in quality improvement initiatives and contribute to a positive and respectful work environment

Requirements:

Current LPN licensure in the state of Nebraska Minimum 1 year of experience as an LPN Certificate in Practical Nursing (LPN) Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams Compassionate and empathetic approach to patient care, with a focus on promoting patient dignity and well-being
