LPN Job Summary A compassionate and dependable Licensed Practical Nurse (LPN) to join our team on the night shift . This position provides nursing care to residents in a long-term care and rehabilitation setting while working closely with residents, families, nursing staff, and other healthcare professionals. The ideal candidate is dedicated to providing high-quality, resident-centered care and is comfortable working independently during overnight hours. Responsibilities Provide direct nursing care to residents in accordance with established care plans and facility policies. Administer medications and treatments as ordered by physicians and according to applicable regulations. Monitor residents' conditions and promptly report changes to the appropriate healthcare provider. Perform nursing assessments and document care accurately and timely. Assist with activities of daily living and promote residents' comfort, dignity, and independence. Communicate effectively with residents, families, and members of the interdisciplinary healthcare team. Follow infection-control, safety, and facility procedures. Respond appropriately to resident needs and emergencies during the night shift. Maintain accurate and complete nursing documentation. Support a positive, respectful, and safe environment for residents and staff. Qualifications Current and valid LPN license in the state of New Hampshire. Graduate of an approved practical nursing program. Long-term care, rehabilitation, or skilled nursing experience preferred. Strong communication, organization, and clinical skills. Ability to work independently and as part of a healthcare team. Compassionate and professional approach to resident care. Ability to work 12-hour night shifts on a variable schedule.
