Your expertise will support the health needs of geriatric and disabled populations, working across various care settings including skilled nursing facilities, assisted living communities, and hospice environments. Responsibilities Deliver comprehensive nursing assessments and develop individualized care plans tailored to each resident's needs. Administer medications, including injections and IV infusions, following strict protocols and safety standards. Monitor vital signs, perform physical examinations, and evaluate patient responses to treatments in accordance with long-term care regulations. Manage complex cases involving dementia, Alzheimer's disease, memory care, and behavioral health issues with sensitivity and professionalism. Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and palliative or hospice care services. Maintain accurate medical documentation within Electronic Health Record (EHR) systems such as Epic or Cerner while ensuring compliance with HIPAA confidentiality policies. Provide patient education on medication administration, catheter care, nutrition, and other health coaching topics to promote independence and quality of life. Skills Proven experience working with geriatric patients in skilled nursing facilities or long-term care environments. Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/ICD-10). Proficiency with EMR/EHR systems such as Epic, Cerner or Athenahealth for documentation and patient management. Skilled in IV insertion, catheterization, sterile processing techniques, and basic life support (BLS). Ability to perform comprehensive patient assessments including vital signs, intake/output monitoring, and diagnostic evaluation. Experience working with individuals with disabilities or developmental delays; familiarity with dementia and Alzheimer's care is highly valued. Excellent communication skills for effective caregiver communication and client interaction across diverse populations.
