What You'll Do As a Home Health LPN, you'll provide skilled nursing care under the direction of a Registered Nurse while helping patients achieve their health goals in the comfort of their own homes.
Administer medications, treatments, and wound care.
Monitor patient conditions and communicate changes to the supervising RN.Educate patients, families, and caregivers to safely manage care at home.
Collaborate with physicians and the interdisciplinary care team to ensure the highest quality outcomes.
Complete accurate, timely documentation using our electronic medical record.
Build meaningful relationships with patients while providing compassionate, personalized care. What We're Looking For Current Licensed Practical Nurse (LPN) license in New Hampshire or Maine (Compact License accepted).Current driver's license, reliable transportation, and automobile insurance.
Current CPR certification (or ability to obtain upon hire).Excellent communication and organizational skills.
Two years of acute care experience preferred.
