A Day in the Life No two days are exactly alikeand that's one of the best parts of Home Health nursing. You'll start your day by reviewing your schedule before heading out to visit patients throughout the Hampton NH community. Rather than rushing from room to room, you'll have the opportunity to spend meaningful one-on-one time with each patient, providing skilled nursing care, education, encouragement, and support. Throughout the day you'll: Care for patients in the comfort of their own homes.

What You'll Do As a Home Health LPN, you'll provide skilled nursing care under the direction of a Registered Nurse while helping patients achieve their health goals in the comfort of their own homes.
Your responsibilities will include:
Deliver skilled nursing visits based on individualized plans of care.

Administer medications, treatments, and wound care.

Monitor patient conditions and communicate changes to the supervising RN.Educate patients, families, and caregivers to safely manage care at home.

Collaborate with physicians and the interdisciplinary care team to ensure the highest quality outcomes.

Complete accurate, timely documentation using our electronic medical record.

Build meaningful relationships with patients while providing compassionate, personalized care. What We're Looking For Current Licensed Practical Nurse (LPN) license in New Hampshire or Maine (Compact License accepted).Current driver's license, reliable transportation, and automobile insurance.

Current CPR certification (or ability to obtain upon hire).Excellent communication and organizational skills.

Two years of acute care experience preferred.
