Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with medical procedures such as catheterizations, suturing, tube feedings, and sterile dressing changes using aseptic techniques. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient care accurately and efficiently. Support patient education on health management, medication adherence, and post-discharge care plans. Collaborate with physicians, specialists, and interdisciplinary teams to coordinate patient care across settings such as hospitals, clinics, long-term care facilities, and home health environments. Manage emergency situations including airway management, acute pain control, and triage assessments with confidence and professionalism. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Proven experience in diverse clinical settings such as critical care units (ICU), emergency rooms, nursing homes, assisted living facilities, or home care environments. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation and patient records management. Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding where applicable, and infection control protocols. Experience with specialized areas including Alzheimer's/dementia/memory care or working with individuals with developmental disabilities is highly valued. Ability to perform venipuncture, phlebotomy, catheter care, medication administration via injections or IV insertion; basic life support (BLS) certification required. Excellent communication skills to effectively assess patients' needs and collaborate with healthcare teams. Commitment to HIPAA compliance and maintaining patient confidentiality at all times.
