Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, skilled nursing services across diverse settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your energetic approach and attention to detail will ensure that patients receive top-quality care in accordance with all regulatory standards and best practices. This position offers an exciting opportunity to utilize your clinical skills while fostering meaningful connections with patients and their families. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to determine individual needs. Administer medications, injections, IV infusions, and other treatments following strict protocols and physician orders. Manage complex care procedures such as catheterization, tube feeding, wound suturing, and ventilator management with precision and confidence. Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic, Cerner, or Athenahealth to ensure seamless medical documentation. Collaborate with interdisciplinary teams to develop and implement personalized care plans that address acute pain management, dementia care—including Alzheimer's and memory care—and chronic conditions. Provide patient education on medication administration, health coaching, post-discharge instructions, and lifestyle modifications to promote recovery and wellness. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and patient information. Experience Valid state licensure as a Licensed Practical Nurse with current CPR/BLS certification. Demonstrated experience working in diverse healthcare environments such as hospitals (including ICU, Level I & II Trauma Centers), nursing homes, skilled nursing facilities, or outpatient clinics. Proven proficiency in EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate documentation and data management. Prior experience in critical care settings such as emergency rooms, ICU units, or trauma centers is highly desirable. Knowledge of ICD coding (ICD-9/10), CPT coding for procedures, and familiarity with long-term care regulations enhances your ability to deliver comprehensive patient care. Experience working with geriatric populations or individuals with disabilities—including dementia or behavioral health challenges—demonstrates your adaptability and compassion. Strong understanding of physiology, anatomy, infection control practices, medication administration protocols—including injections and venipuncture—and sterile techniques to ensure safe patient outcomes.
