The role emphasizes patient care, medication administration, and maintaining safety while fostering collaboration and communication with families. Responsibilities Deliver direct patient care throughout the day Administer medications accurately and timely Maintain detailed daily documentation of patient status and activities Monitor health conditions and report changes promptly Ensure safety supervision for all participants Collaborate effectively with DDD rules and regulations and the broader care team Communicate regularly with families regarding patient progress Respond promptly and appropriately to emergencies Required Qualifications Valid LPN license Proficient in medication administration Strong patient care techniques Effective communication skills Excellent time management abilities Teamwork and collaboration skills Keen observation skills Preferred Qualifications Minimum of 2year experience in licensed practical nursing Formal LPN certification or diploma
