Licensed Practical Nurse (LPN) - Wellness Nurse We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our wellness team. The Wellness Nurse will play a vital role in promoting health and well-being among our clients through preventive care, health education, and routine nursing services. Key Responsibilities Provide routine nursing care and wellness assessments to clients. Assist in developing and implementing individualized wellness plans. Monitor vital signs and document patient health status accurately. Administer medications and treatments as prescribed by healthcare providers. Educate clients on healthy lifestyle choices, disease prevention, and self-care techniques. Collaborate with healthcare professionals to coordinate care and follow-up. Maintain accurate and confidential patient records in compliance with regulatory standards. Respond promptly to client concerns and provide appropriate interventions. Qualifications Current and valid Licensed Practical Nurse (LPN) license. Proven experience as an LPN, preferably in a wellness or preventive care setting. Strong knowledge of nursing practices, medication administration, and patient care. Excellent communication and interpersonal skills. Ability to educate and motivate clients towards healthier lifestyle choices. Detail-oriented with strong organizational skills. Ability to work independently and as part of a multidisciplinary team. Work Environment This position may involve working in clinical settings, community centers, or client homes, depending on organizational needs. Flexibility and adaptability are essential.
