Evening and Night Shift Per Diem Pay:
$33-$52.33/hour Compensation based on license, experience, shift, and weekend differentials. LPN:
Perform resident assessments and collect clinical data to support individualized care planning Provide and receive shift reports to ensure accurate and consistent continuity of care Administer and document medications, treatments, and enteral nutrition in accordance with physician orders, following the 6 rights of medication administration Complete timely and accurate clinical documentation, including nurses' notes, MDS/RAPs, and incident/accident reports Coordinate resident admissions, transfers, and discharges, ensuring all required documentation is completed accurately Collaborate with residents, families, and interdisciplinary care teams to support overall health, safety, and well-being What You'll Do as an
RN:
Conduct comprehensive nursing assessments and monitor residents' health status, promptly identifying and reporting changes Develop, implement, and participate in resident care plans and care plan meetings Manage resident admissions, transfers, and discharges, ensuring all required documentation is completed accurately Administer and document medications, treatments, and enteral nutrition per physician orders, adhering to the 6 rights of medication administration and maintaining accountability for narcotics and medication keys Complete accurate and timely documentation in the EHR, including nurses' notes, MDS/RAPs, and incident/accident reports Collaborate with residents, families, healthcare partners, and community resources to ensure continuity of care Qualifications Active New Jersey Licensed Practical Nurse (LPN) or Registered Nurse (RN) license Why Join UMC?
