Job Summary:
You will be responsible for delivering to clients comprehensive nursing services utilizing the nursing process. Additionally, you will be responsible for interventions that promote and foster health, assess dysfunction, assist clients to regain or improve their coping abilities, and prevent further disability.
What You Will Do :
Responsible for client health promotion and health maintenance Complete intakes, assessments and evaluations Provide case management services Provide to clients a therapeutic environment Track clients and assisting them with self-care activities, administering and monitoring psycho-biological treatment regimens, health teaching, crisis intervention, counseling, and outreach activities such as home visits Conduct health examinations, including the monitoring of vital signs Maintain registry of health screening data, (glucose, lipids, blood pressure, weight, BMI, blood work) Monitor health status of participants longitudinally Reports health status of participants to the treatment team Participate in the development of the participant's recovery plan Coordinate all medical care services, link consumers with specialty care and provide supports in accessing the care Provide psycho-education and wellness related services Function as an embedded member of an interdisciplinary team Document services per Agency policy and federal grant contract requirements What We Need to
See :
High school degree or equivalency plus one (1) year experience working with a psychiatric population A valid New Jersey LPN license CPR certified A valid driver's license and must be insurable by the agency's automobile insurance.
