Job Description

Job Summary Join our dedicated home care team as a Licensed Practical Nurse (LPN) or Registered Nurse (RN), where you will deliver compassionate, high-quality nursing care directly in patients' homes. This vital role offers the opportunity to make a meaningful difference in individuals' lives by providing personalized health services, managing complex medical needs, and supporting independence. You will work closely with patients, families, and healthcare teams to ensure safe, effective, and respectful care tailored to each person's unique circumstances. Your expertise will help promote health, comfort, and well-being in a familiar environment. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications, injections, IV infusions, and perform catheterization with precision and adherence to safety protocols. Develop and implement individualized care plans that incorporate pain management, dementia care, Alzheimer's care, hospice support, and other specialized services. Manage medical documentation accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic or Cerner. Provide education and coaching on health maintenance, medication administration, nutrition, and disease management tailored to seniors and individuals with disabilities. Perform complex procedures including sterile techniques like phlebotomy, suturing, spinal taps, and gastrostomy tube feeding. Collaborate with interdisciplinary teams for discharge planning, case management, and coordination of post-acute or sub-acute care services. Ensure compliance with HIPAA regulations and infection control standards while maintaining patient confidentiality at all times. Assist with emergency situations such as airway management or acute pain episodes in home settings or during hospital visits. Experience Valid LPN or RN license with current state licensure required. Critical care experience preferred; including ICU or Level I Trauma Center exposure demonstrating ability to handle high-acuity cases. Experience working with individuals with developmental disabilities, dementia (including Alzheimer's), or memory care needs is highly valued. Proven background in home & community care environments along with long-term care experience such as nursing homes or assisted living facilities. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is essential for accurate documentation. Knowledge of
ICD-9/10
coding for diagnoses and CPT coding for procedures enhances clinical documentation accuracy. Prior experience in hospice care, palliative medicine, or acute pain management is advantageous. Ability to perform patient assessments involving physiology knowledge and diagnostic evaluation techniques including specimen collection/processing and medical imaging understanding.
