Rotating monthly after-hours on-call schedule Join Our ACT Team! Southern Highlands Community Mental Health Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) team. This position provides community-based nursing services to individuals with serious mental health needs, helping them achieve stability, independence, and improved quality of life. As a member of the ACT team, the LPN works collaboratively with the Registered Nurse and multidisciplinary treatment team to provide nursing support, medication management, health education, and crisis intervention services in home and community settings. The Licensed Practical Nurse (LPN) assists with daily medication dispensing and delivery, monitors medication compliance and effectiveness, and provides health education and support to consumers and their families. This position coordinates referrals to medical providers and community resources, assists with crisis intervention and support services, and maintains accurate documentation of services provided. The LPN works collaboratively as a member of the ACT multidisciplinary team, delivering community-based services in consumers' homes and other community settings while participating in a rotating monthly after-hours on-call schedule. Qualifications Current Licensed Practical Nurse (LPN) license in the State of West Virginia required. Strong organizational, communication, and interpersonal skills. Ability to work independently in community settings and as part of a multidisciplinary team. Valid driver's license and reliable transportation.
