09/12/2026 Job Description Job Description Job Summary Senior Spirit of Roselle Park, LLC Adult Medical Day Center - is s eeking Full Time, Bi-Lingual Spanish/English speaking, highly motivated LPN Nursing join our team. In this role, you will be under the direction of the Director of Nursing, the LPN shall perform functions which shall enable the Members to perform their ADL's. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong ability for solving problems. Previous experience working in elder care and with disabled individuals would be highly beneficial. Responsibilities Prepare and update spreadsheets for tracking purposes - New/Transfer Visitors. Communicate with State Agencies in regards to Nursing Assessment Visits, Prior Authorizations. Create charts from files for new members, charts for current members and communicate with staff regarding members. Discharge members in accordance with SSRP guidelines. Assist with translation/assistance for members/visitors. Assist with coordinating doctor appointments and transportation. Monitor Members activities of daily living. Document any changes in patient conditions and report on changes. Maintain accurate records/charts of patient care and discuss observations with supervisor. Assist with daily vitals.
