As a Licensed Practical Nurse (LPN) at Cornell Care & Rehabilitation Center managed by Autumn Lake Healthcare at Union, you will play a vital role in providing exceptional care to our patients. Our clinical excellence and compassionate care philosophy ensures that every patient receives personalized attention and treatment. We are seeking a dedicated and compassionate LPN to join our team of specialty caregivers.

Responsibilities:

Provide direct patient care, including administering medications, performing vital signs, and documenting patient information Develop and implement individualized patient care plans in collaboration with the interdisciplinary team Monitor and report patient progress to the healthcare team and families Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care Provide emotional support and comfort to patients and their families Maintain accurate and up-to-date patient records Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
