Job Description

  • _ o hours are guaranteed for this position. Scheduling will be on an as-needed basis, depending on when the family requires respite care services

  • Currently licensed as an LVN/LPN by the state.
  • At least one year of experience in capacity of an LVN/LPN with in the last three years.
  • CPR desirable
  • Must be able to lift a minimum of 50 pounds.
Home Health Care experience desirable Ability to use diplomacy Ability to communicate efficiently and effectively both verbally and written
Key Responsibilities:
  • Observes, records and reports to the Registered Nurse on the general physical and mental condition of the client.
  • Administers those prescribed treatments as permitted by applicable state law.
  • Assists the physician and/or RN in performing specialized procedures.
  • Prepares and completes clinical and progress notes submitting them at least weekly.
  • Prepares equipment for treatment.
  • Assists patient with Activities of Daily Living (ADL) and includes appropriate self-care.
  • Participates in team conferences, patient care audit and other committees as requested.
  • All other duties as assigned
  • Uphold company policies and promptly report any misconduct
Physical & Essential Job Functions:
  • Ability to communicate effectively through speaking and hearing
  • Must be able to sit for long periods and utilize hands for tasks such as typing, writing, and handling objects
  • Regular attendance and punctuality are required
  • Must be able to stoop, kneel, crouch, stand, and walk as needed
  • Ability to lift, carry, push, or pull objects weighing up to 50 pounds
  • Vision requirements include the ability to see clearly at close range and maintain focus on detailed tasks.
  • Mental capabilities must include the ability to understand and apply both oral and written instructions, demonstrate strong memorization skills, and engage in effective problem-solving.
