Setting:

Skilled Nursing Facility (Long-Term Care) Responsibilities Administer medications and treatments according to physician orders and facility policy. Monitor patient condition, document vital signs, and report changes to the supervising RN promptly. Assist residents with activities of daily living and mobility, establishing a safe and supportive care environment. Perform wound care, dressing changes, and routine patient assessments as directed. Collaborate with registered nurses, physicians, and other healthcare team members to support individualized care plans. Maintain accurate and timely documentation in patient records and ensure compliance with regulatory requirements. Ability to work in a long-term care setting and participate in a travel assignment.
