$1,450•$1,522 (Based on Experience) Position Overview We are seeking a dedicated Licensed Practical Nurse (LPN/LVN) to join a long-term care team providing compassionate nursing care to residents in a skilled nursing and memory care setting. This role focuses on delivering high-quality care to residents, including those with Alzheimer's disease and dementia, while collaborating with an interdisciplinary healthcare team to support patient well-being and safety. Responsibilities Provide direct nursing care to residents in accordance with individualized care plans. Administer medications, treatments, and therapeutic interventions as prescribed. Monitor resident conditions and promptly report significant changes to the appropriate healthcare providers. Maintain accurate and timely patient documentation using electronic medical records. Assist with admissions, transfers, and discharge processes as needed. Collaborate with physicians, nurses, therapists, and support staff to ensure coordinated resident care. Support residents with activities that promote comfort, dignity, and quality of life. Respond appropriately to resident needs, emergencies, and changes in condition. Adhere to facility policies, nursing standards, and regulatory requirements. Participate in quality improvement and patient safety initiatives. Required Qualifications Education Graduate of an accredited Practical Nursing Program. Licensure & Certifications Active New York State LPN License (license must be in good standing). Current BLS Certification. Experience Minimum of one (1) year of Long-Term Care experience preferred. Candidates with recent experience in rehabilitation, medical-surgical, or hospital settings will also be considered. Experience caring for residents with Alzheimer's disease and dementia preferred. Additional Requirements Proof of active nursing license required at submission. First-time travelers are welcome to apply. Local candidates accepted. Requested Time Off (RTO) must be submitted and approved prior to offer acceptance. Ability to work independently during night shifts while maintaining excellent patient care standards. Strong communication, documentation, and teamwork skills.
