Job Description

Join our dedicated healthcare team as a Licensed Practical Nurse (LPN), where you will play a vital role in delivering compassionate, high-quality patient care across diverse settings. In this dynamic role, you'll utilize your clinical expertise to assess patient needs, administer treatments, and support recovery and wellness initiatives. Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions and specialized treatments like botulinum toxin or dermal fillers, following strict protocols and HIPAA regulations. Assist with diagnostic procedures such as venipuncture, catheterization, spinal taps, and specimen collection/processing to support accurate medical evaluation. Provide wound care, sterile processing, and aseptic techniques during procedures like suturing and dressing changes to promote optimal healing. Support patients with complex needs such as dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Educate patients and families on health coaching, medication administration, discharge planning, nutrition, and post-acute care strategies to empower self-care and recovery. Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Strong knowledge of physiology, anatomy, medical terminology, ICD-9/10 coding standards for precise patient assessment and record keeping. Critical care experience including ICU or emergency room settings; experience in trauma centers or acute care environments is highly valued. Skilled in IV insertion/insertion techniques (IV infusion), catheter care, tube feeding management and ventilator support when applicable. Ability to perform physical examinations and diagnostic evaluations confidently while adhering to infection control protocols.
