Job Description
Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality, patient-centered nursing care across various settings, including long-term care, assisted living, hospitals, and community health environments. This role involves performing comprehensive patient assessments, administering medications and treatments, and supporting the overall health and well-being of diverse patient populations. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional care in accordance with regulatory standards and organizational policies. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications, injections—including IV infusions—and treatments accurately following physician orders and safety protocols. Monitor patients for changes in condition, managing acute pain and providing appropriate interventions. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and ventilator management. Maintain detailed medical documentation within Electronic Health Records (EHR) systems while ensuring compliance with HIPAA and confidentiality policies. Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and long-term care strategies. Provide patient education on medication administration, disease management—including Alzheimer's and dementia care—and health coaching. Support infection control practices and sterile processing procedures to maintain a safe environment. Engage in triage activities by assessing urgent needs and prioritizing care delivery effectively. Work with diverse populations including geriatric patients, individuals with developmental disabilities, pediatric patients—including neonatal and toddler care—and those requiring specialized services such as hospice or infectious disease management.
