Job Description

Clinical/Technical Responsibilities Assists the patient in the achievement of identified goals by carrying out the prescribed plan of care. Demonstrates patient advocacy by assuring a patient's right to privacy, safety and confidentiality. Maintains current knowledge and skills by participating in relevant educational and training programs. Ability to flexibly respond to changing demands. Participates in the daily huddle where the schedule for the day will be reviewed and follow-up tasks will be assigned. No shows and same day cancellations from previous business day will be discussed and follow-ups will be assigned. Conducts pre-visit preparations. The practice asks patients (e.g., by letter or portal) to complete required paperwork before a scheduled visit, in addition to lab tests, imaging tests or referral visits. The practice reviews test results before the visit in the team huddle. Meets environmental responsibilities as assigned that may include setting up instruments and equipment according to department protocols; cleaning exam/procedure rooms, instruments, and equipment between patient visits to maintain infection control; ordering, sorting, storing supplies; and restocking exam/procedure rooms. Fulfills clinical responsibilities, which include checking vital signs; physical examination preparations; prescription verifications with physician's orders; biohazard waste disposal and monitoring; laboratory procedures including Occupational Safety and Health Administration (OSHA) guidelines; quality control methods; CLIA-waived testing; venipunctures; specimen handling such as urine, throat, vaginal, stool, and sputum; EKG testing; completing lab draws; emergency first aid; administering immunizations and injections Monitor vaccine, medication and laboratory supplies; coordinating with CMH hospital departments as appropriate Clinic Stakeholder Responsibilities Participates effectively as a member of the healthcare team. Treats patients, families, visitors and co-workers with courtesy and respect. Provides direct constructive feedback to colleagues as appropriate. Clinic Financial Responsibilities Demonstrates effective time management skills Participates in performance improvement activities by identifying opportunities for improvement, offering solutions and supporting changes. Uses resources efficiently. Utilizes Community Memorial Hospital's property and supplies appropriately and prudently to maximize cost effectiveness of procedures, conserve resources and minimize waste Reviews e-bills and captures all services performed Clinic Health Information Management Responsibilities Offers and receives feedback on own performance and on the quality of patient care delivered. Uses feedback to improve practice and patient care. Consistently ensures that patient information is kept private and confidential.
Competencies:
Education Graduation from an accredited Practical Nursing program is required. Knowledge Knowledge from previous nursing experience is desirable. Knowledge of basic nursing principles and protocols. Knowledge of policies and procedures related to infection control, environmental safety, pain management, emergency situations, fire safety, performance improvement and patient confidentiality. Knowledge of Electronic Medical chart, Meaningful use reports as well as Performance Improvement reports. Skill in appropriate assessment and assistance techniques. Skill in appropriate use of universal precautions, safe workplace and confidentiality methods. Skill in health information management by appropriately charting patient data. Required Licensure/Certification/Credentials Current NYS licensure as a Practical Nurse is required. Current CPR certification is required.
