Responsibilities:

Provide direct patient care to residents, including administering medications, performing wound care, and assisting with daily living activities Monitor and report changes in resident condition to the nursing team and medical staff Collaborate with interdisciplinary team members to develop and implement individualized care plans Maintain accurate and up-to-date resident records and reports Develop and maintain positive relationships with residents, families, and colleagues Participate in quality improvement initiatives and maintain a safe and clean environment
