Independence Home Care of New York, Inc. was created to become a very unique provider of high quality residence-based home care services. As a home care agency, the majority of our Patients are with us because, although they may not require nursing home care, they do need assistance with a great amount of ADL support. Our goal will be to keep this population safe, healthy, happy and aging in place! We are looking for a reliable LPN (Licensed Practical Nurse) to care for the aging population in a professional and caring manner. You will work under the supervision of a registered nurse.. LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding.

5:00am

• 1:00pm 6:00pm
• 2:00pm 2:00pm
• 10:00pm
• Shift Differential Offered All shifts are Monday through Friday, every other weekend Responsibilities This position does direct care alongside HHA's Record a patient's medical history accurately Take and record measurements of blood pressure, temperature, heart rate etc.

Observe patients under treatment to identify progress, side-effects of medications etc. Monitor patients' condition including fluid intake and output and compose patient charts Assist ailing patients in daily necessary activities such as eating, bathing etc. Collect samples for lab testing as assigned Skills Proven experience as LPN or similar role Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team Familiarity with Home health, safety and sanitation standards and procedures Understanding of confidentiality obligations and nursing best practices Excellent communication and interpersonal skills Compassionate and able to handle stress Successful completion of LPN license program is a must Benefits offered: Paid time off Health insurance
