Licensed Practical Nurse (LPN) - Treatment Coordinator Freeport, NY ATC Healthcare Services, LLC of Nassau County has an
IMMEDIATE NEED
for an experienced Licensed Practical Nurse (LPN) Treatment Coordinator to work in a Skilled Nursing and Rehabilitation Facility located in Nassau County, NY.
Schedule:

Administer medications, treatments, and nursing interventions in accordance with physician orders and facility policies. Monitor patient conditions and promptly report changes to physicians, Registered Nurses, and other healthcare professionals. Perform routine assessments and document patient responses to treatment. Review and update treatment records, care plans, and medical documentation. Schedule and coordinate appointments, consultations, diagnostic testing, and follow-up care. Communicate treatment goals and patient progress with residents, families, and healthcare team members. Assist with wound care, dressing changes, catheter care, and other clinical procedures as assigned. Ensure compliance with infection control standards, patient safety protocols, and regulatory requirements. Participate in interdisciplinary care conferences and contribute to care planning discussions. Educate patients and families regarding treatment plans, medications, and health management strategies. Maintain confidentiality of patient information in accordance with HIPAA regulations. Assist with quality assurance and performance improvement initiatives. Qualifications Active New York State Licensed Practical Nurse (LPN) License required. Current CPR/BLS Certification required. Minimum one (1) year of nursing experience preferred. Experience in long-term care, rehabilitation, home care, outpatient, or clinical settings preferred. Knowledge of medical terminology, treatment protocols, and nursing practices. Proficiency with Electronic Medical Records (EMR/EHR) systems. Strong organizational, communication, and interpersonal skills. Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment. Excellent critical thinking and problem-solving skills.
