Job Description
Humancare Home Healthcare Services is a certified home healthcare agency. We are seeking qualified and experienced Licensed Practical Nurses (LPNs) to provide skilled, one-on-one care to adult patients in a homecare setting. These homecare opportunities are ideal for nurses with strong high-acuity experience who are committed to delivering exceptional patient care. We are currently looking for LPNs to provide homecare nursing services for the following locations > Jamaica Queens and Long Island City Queens for day or nights shifts 12 per day on an on-going basis. Duties> Deliver skilled nursing care to adult patients in a homecare environment Manage and monitor high-acuity cases requiring advanced clinical skills Perform specialized care including: Tracheostomy (Trach) Care Ventilator Management Gastrostomy Tube (G-Tube) Care Nasogastric Tube (NGT) Management Administer medications and treatments in accordance with physician orders Monitor patient status and report changes promptly Maintain accurate and compliant clinical documentation Collaborate with interdisciplinary care teams to ensure optimal patient outcomes Compensation & Benefits Highly Competitive Pay Rates (based on experience and case acuity) Weekly Pay with Direct Deposit Referral Incentives Available Flexible Scheduling Options (Day & Night Shifts) Consistent Case Assignments for Continuity of Care Overtime Opportunities Ongoing Training and Clinical Support Active New York State Licensed Practical Nurse (LPN) License Demonstrated experience in homecare with adult patients Required clinical competencies: Trach Care Ventilator Care G-Tube and NGT Management Ability to work independently in a patient home setting Strong clinical judgment, reliability, and professionalism Qualified candidates are encouraged to apply for immediate consideration.
