8-hour blended Mid & Night shifts Job Summary Facility is seeking an experienced Licensed Practical Nurse (LPN) to join the Skilled Nursing/Long-Term Care team. The LPN will provide safe, compassionate, and quality nursing care to residents while working collaboratively with the healthcare team. Responsibilities Provide direct nursing care to residents in a skilled nursing and long-term care setting. Administer medications and treatments according to physician orders and facility policies. Monitor residents' conditions and report changes to the appropriate healthcare provider. Perform assessments, vital signs, and routine nursing procedures. Maintain accurate and timely clinical documentation. Assist residents with activities of daily living and promote their comfort, dignity, and independence. Communicate effectively with residents, families, physicians, and other members of the healthcare team. Follow infection control, safety, and facility protocols. Respond appropriately to changes in residents' health status and emergency situations. Collaborate with RNs, CNAs, and other healthcare professionals to ensure continuity of care. Requirements Active New York LPN License Current
BCLS/BLS
Certification Minimum 2 years of recent Long-Term Care (LTC) experience Skilled Nursing/LTC experience required Strong clinical, communication, and organizational skills Ability to work Mid and Night shifts as scheduled
