POSITION SUMMARY
The Licensed Practical Nurse (LPN) supports efficient, high-quality patient care primarily through centralized follow-up and care coordination, with additional in-clinic support on ICA and procedure days as requested. Under the direction of the RN and/or Provider, the LPN is responsible for post-visit follow-up, patient outreach, documentation, education, and coordination of care related to family planning, gender affirming care, reproductive health, abortion services, and STI care. This role also provides procedural assistance during designated clinic days and exemplifies PPNCNY's commitment to teamwork, compassion, confidentiality, quality care, and mission.
ESSENTIAL DUTIES
Primary Responsibilities - Centralized Follow-Up 1. Conduct centralized follow-up with patients, including reviewing results, providing education, coordinating next steps, and responding to patient questions. 2. Communicate patient needs, concerns, and follow-up requirements to providers and clinical leadership. 3. Document all follow-up interactions accurately and timely in the medical record in accordance with PPNCNY, PPFA, and DOH standards. 4. Retrieve, transfer, and manage patient medical records for follow-up care, referrals, audits, and quality management as required. 5. Schedule follow-up appointments and coordinate care according to health center policies and capacity guidelines. 6. Provide information to patients via phone or electronic communication following health center policies and confidentiality standards. 7. Facilitate patient refill requests by phone message, patient portal messages, from pharmacies, and via direct requests from patients Secondary Responsibilities - In-Clinic & Procedural Support 9. Assist with ICA, abortion, colposcopy, LEEP, vasectomy, and other procedures on designated clinic days as requested. 10. Support providers during exams and procedures, including preparation, assistance, and documentation. 11. Perform medication administration, including injections, and related documentation per clinical standards. 12. Perform lab functions as needed, including specimen collection, processing, and preparation for transport. 13. Conduct macroscopic exams of tissue post-abortion and manage specimen documentation, reporting, and follow-up as required. Operational & Team Support 14. Assist with all day support staff training education days 15. Support patient flow and clinic operations during in-clinic days as assigned. 16. Maintain infection control, safety, and quality control standards. 17. Collaborate with health center and affiliate staff to ensure excellent internal and external customer service. 18. Serve as a preceptor to newly hired Medical Assistants or LPNs, as assigned. 19. Attend required staff meetings and trainings. 20. Adhere to all PPNCNY policies related to professionalism, confidentiality, safety, and patient privacy.
Requirements:
REQUIRED SKILLS AND ABILITIES
1. Understands the importance of maintaining confidentiality; able to maintain confidentiality under HIPAA standards. 2. Must have the ability to exercise a high degree of professionalism; excellent customer services and interpersonal communication skills; Cultural sensitivity and demonstrated ability to work with diverse people groups. 3. Able to effectively relate via the telephone and in-person to serve the needs of health center patients/clients/visitors in a manner that is efficient and productive. 4. Must be highly flexible; able to accommodate changing needs of the department. 5. Excellent verbal and written communication skills. 6. Ability to work well under pressure with minimal supervision. Proven flexibility and willingness to handle a variety of tasks. 7. Willingness to work evenings and/or weekends. 8. Basic to intermediate computer knowledge and skills. 9. Ability to travel to other centers as needed. 10. Valid Driver's License 11. Assists in training new employees. 12. Acceptance and understanding of PPNCNY's Personnel Policies.
QUALIFICATIONS
1. Licensed Practical Nurse in New York State. 2. Family planning/GYN experience preferred. 3. Current certification in
CPR/BLS. PHYSICAL DEMANDS/WORKING CONDITIONS 1.
