A Licensed Practical Nurse or Vocational Nurse provides basic medical care under the direction of registered nurses and other healthcare specialists. The tasks that they are permitted to perform vary by state, but usually include changing bandages, checking blood pressure, or helping feed patients. May work in hospitals, nursing homes, or other healthcare facilities.
Assist providers with outpatient family medicine and obstetric patient care. Room patients, obtain vital signs, document medical histories, and prepare patients for examinations and procedures. Perform clinical tasks including immunizations, injections, point-of-care testing, EKGs, specimen collection, and medication administration within scope of practice. Support prenatal and postpartum visits, newborn care, and patient education. Coordinate referrals, diagnostic testing, and follow-up care. Communicate with patients regarding test results, medication questions, and care plans. Collaborate closely with physicians, advanced practice providers, medical assistants, patient service representatives, and the care management team to deliver exceptional patient-centered care. Demonstrates accountability for the provision of nursing care Assists with provider documents ("folder manager") Collects data and communicates patient information in order to optimize patient outcomes Respects cultural customs and beliefs Assists in determining the health status and health needs of the patient and assists in the formulation of goals/outcomes Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient Assists in the coordination of human and material resources for the provision of care to assigned patients Participates with other members of the health care team for patient care delivery Demonstrates professional growth as evidenced by a minimum of ten(10) hours of related education annually Responsible for working with providers to best efficiently meet their needs and our patients' needs Assists with procedures as appropriate and needed Provides stocking, storage, cleaning, communication nurse duties and others as assigned Responsible for NYSDOH Rx EMR paper Ensures safety of all medicines and medical supplies maintained at the practice site
EDUCATION/EXPERIENCE
Graduate of an accredited school of Practical Nursing Current New York state, LPN registration or eligible for licensure required Current Pennsylvania LPN license preferred Successful completion of BLS course required This position requires experience in all areas of nursing (primary care, OB, etc.) Excellent communication and organizational skills.
