Licensed Practical Nurse (LPN) - Rotterdam PACE Medical Clinic - Eddy SeniorCare Monday-Friday | No Nights | No Weekends This is not a typical LPN job—and that's the point! No rotating shifts Clinic‑based work with occasional home visits Time to actually know the participants—not just their diagnoses Supportive RN Case Managers and Providers who value your input Mission‑driven, nonprofit care model focused on quality Strong benefits, paid time off, and stable schedules About the Role This LPN role is primarily based in our Rotterdam PACE Medical Clinic, with occasional community visits for medication support, wound care, lab draws, or other follow‑up needs. You'll provide skilled nursing care to older adults enrolled in the PACE program—helping them remain safely at home while avoiding unnecessary hospitalizations or nursing home placement.You will work closely with:
Physicians RN Case Managers Social Workers Therapists and Home Care staff What You'll Do Day‑to‑Day In the Clinic Room participants and prepare them for provider visits Administer medications, injections, vaccines, and treatments Perform vital signs, documentation, and EMR charting Assist with procedures while ensuring comfort and dignity Manage clinic medications, supplies, and infection control Support phlebotomy, specimen collection, INR checks, and testing In the Community (Occasional) Home visits for mediset changes, wound care, labs, or medication follow‑up Deliver medications or supplies when needed Communicate changes to the RN Case Manager promptly What Makes This Role Different You are not stuck in constant crisis mode You practice at the top of your LPN license You still get hands‑on nursing, without burnout schedules You're part of decisions—not just following orders Your voice matters on the care team What We're Looking For Required Active, unencumbered New York State LPN license Valid driver's license and vehicle Comfort working with older adults Preferred 1+ year experience with geriatric, community, or chronic care populations EMR experience Interest in learning or strengthening skills like phlebotomy or IV therapy (within scope) Who Thrives Here This role is ideal for an LPN who: Values relationships over rushing Enjoys both clinic structure and occasional field work Communicates clearly and compassionately Wants long‑term stability, not shift‑hopping Believes great care includes patience, dignity, and follow‑through About Eddy SeniorCare PACE Eddy SeniorCare is part of St. Peter's Health Partners and Trinity Health. PACE (Program of All‑Inclusive Care for the Elderly) is a nationally recognized model that allows older adults to receive comprehensive medical and social care—while remaining in their homes.Pay Range:
$24.00 - $30.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
