Job Description
CHP Home Care & Hospice is hiring a part-time, reliable, qualified, and compassionate LPN for home health care for a local patient in the St. Henry, Oh. area. Immediate opportunity 8 hours per day/24 hours per week (hours are flexible) Local patient, one-on-one care What You Will Do Deliver in-home nursing care for client on a ventilator. Responsibilities Monitor, observe, and document patient's condition Deliver patient's daily nursing care needs Administer general nursing care tasks, administer medications/treatments Provide emotional and psychological support as needed Communicate with family about patient's care and instructions Skills Proven experience as LPN or similar role Ability to administer a variety of care tasks and work independently Understand confidentiality obligations and nursing best practices Excellent communication and interpersonal skills Requirements Driver's license and reliable, insured, vehicle LPN license from an accredited program Valid Ohio nursing license About Our Company CHP Home Care & Hospice is a regional leader dedicated to providing compassionate home health care and hospice services in northwest and west central Ohio.
