As an LPN on our Day Shift team, you will be an integral part of our healthcare team, working closely with our physicians, therapists, and other healthcare professionals to provide high-quality care. Job Summary We are seeking a compassionate and skilled LPN Day Shift to join our team at Meadow Falls of Conneaut. As an LPN, you will be responsible for providing routine nursing care to our residents, including administering medications, treating wounds, and monitoring vital signs. You will also be responsible for implementing care plans, communicating with residents and families, and maintaining accurate records. Responsibilities Provide direct patient care to residents, including administering medications, treating wounds, and monitoring vital signs Implement care plans, in collaboration with the interdisciplinary team, to address residents' physical, emotional, and social needs Communicate with residents and families, including providing updates on care and responding to concerns Maintain accurate and detailed records of resident care, including administering medications, treatments, and procedures Participate in quality improvement initiatives to ensure the highest level of care for our residents Requirements Current LPN license in the state of Ohio High school diploma or equivalent required 0 years of experience as an LPN (new graduates welcome to apply!)
