This position offers an exciting opportunity to work autonomously while collaborating with interdisciplinary teams to deliver comprehensive home health services. Duties Provide Skilled Nursing services, on a intermittent basis, to patients, in their homes, under the direction of a Registered Nurse, in accordance with physician orders Assist the Registered Nurse in performing specialized procedures, as directed, by the Registered Nurse under physician's orders. Assist with the preparation, implementation, and continuing evaluation of the patient care plan. Assist the interdisciplinary team in carrying out the plan of care Observe, record, and report, to the appropriate person, patient symptoms, reactions, and changes Requirements Valid LPN license in good standing with the state licensing board. Proven experience in home health nursing or related settings such as hospitals, nursing homes, assisted living facilities, or hospice care. Basic life support (BLS) certification required; additional certifications such as IV insertion or advanced wound care are beneficial.
