Job Overview We are seeking a compassionate and skilled Hospice Nurse - LPN/LVN to provide hospice nursing care in patients' homes. This role supports the RN and interdisciplinary team to deliver high‑quality end‑of‑life care for patients. Hospice Nurse - LPN/LVN Key Responsibilities Deliver skilled nursing care in alignment with the patient's plan of care. Assist in assessing physical, psychosocial, and environmental patient needs. Participate in developing and implementing patient care plans. Ensure continuity and quality of care through timely documentation and teamwork. Communicate effectively with patients, families, team members, and referral sources. Participate in interdisciplinary meetings, in‑service training, and quality improvement programs. Follow infection control and safety procedures. Uphold agency policies and professional standards. Hospice Nurse - LPN/LVN Minimum Requirements Graduate of an accredited LPN/LVN program. Current LPN/LVN state license Current CPR certification Valid driver's license, reliable transportation, and current auto insurance. Strong clinical and interpersonal skills. Minimum 2 years of clinical nursing experience; hospice experience preferred. Work Environment Care provided in patient homes under varying conditions. Exposure to infectious diseases, blood, and bodily fluids possible. Requires local travel and ability to work a flexible schedule. May involve prolonged standing, walking, and lifting up to 50 lbs.
