Job Summary We are seeking a dedicated and detail-oriented MDS Coordinator - LPN (Licensed Practical Nurse) to join our healthcare team full-time. In this vital role, you will lead the comprehensive assessment and documentation of patient care needs within long-term care, skilled nursing, or hospital settings. Your expertise will ensure accurate completion of the Minimum Data Set (MDS) assessments, which are essential for quality care planning, regulatory compliance, and reimbursement processes. As an energetic team player, you will collaborate with interdisciplinary staff to optimize patient outcomes while adhering to all relevant healthcare regulations and standards. Responsibilities Conduct thorough MDS assessments in accordance with federal and state regulations, including CMS guidelines and long-term care standards. Review and analyze medical records, clinical documentation, and coding (ICD-10, CPT) to ensure accuracy and completeness of data. Coordinate with nursing staff, physicians, case managers, and other healthcare professionals to gather comprehensive patient information. Ensure timely completion and submission of MDS schedules, including quarterly reviews and annual assessments. Maintain compliance with HIPAA privacy regulations and health information management policies during all documentation activities. Utilize EMR/EHR systems Point CLick Care to document assessments accurately within electronic health records. Participate in quality assurance initiatives related to clinical documentation improvement (CDI), utilization management, and compliance audits. Stay current on Medicare regulations, CMS guidelines, and long-term care policies affecting MDS processes. Assist in discharge planning, ensuring proper documentation supports seamless transitions of care for patients across settings such as hospice or outpatient services. Support training efforts for staff on clinical documentation standards and coding practices to enhance accuracy and compliance. Experience Proven experience as an MDS Coordinator or in a similar role within long-term care facilities, hospitals, or skilled nursing environments. Strong knowledge of medical terminology, anatomy, physiology, and clinical documentation standards. Familiarity with EMR/EHR system, Point Click Care, is essential. Experience with ICD-10 coding, CPT coding, DRGs (Diagnosis Related Groups), and Medicare/Medicaid regulations is required. Prior Long-term Care/Skilled is highly desirable. Knowledge of health regulation policies including CMS guidelines, NCQA standards, HIPAA compliance, and long-term care regulations. Ability to review medical records thoroughly for documentation review and ensure adherence to clinical documentation improvement initiatives. Strong organizational skills with the ability to manage multiple assessments efficiently while meeting deadlines. Excellent communication skills for collaborating across multidisciplinary teams and providing education on documentation practices.
