LPN 7-7 Days and Nights Senior Suites - 2.4 Broken Arrow, OK Job Details Full-time 1 day ago Qualifications LPN Full Job Description We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team in our skilled nursing and long-term care facility, providing high-quality, person-centered care to residents in a safe, supportive environment. To be effective in this role, LPNs should possess strong observational skills, empathy, and the ability to communicate effectively with both patients and healthcare teams. They must also be able to work under pressure and adapt to the unique challenges of long-term care settings Key Responsibilities of LPNs in
LTC and SNF Patient Care:
provide essential care to residents, including monitoring vital signs, assisting with daily activities (such as bathing, dressing, and eating), and ensuring proper nutrition and hydration.
Medication Administration:
administer prescribed medications and treatments, ensuring that residents adhere to their medication schedules.
Health Monitoring:
regularly check on residents' health, observing any changes in their condition and reporting these to physicians.
Collaboration:
work closely with RNs, physicians, and other healthcare professionals to develop and implement individualized care plans for residents. They also supervise Certified Nursing Assistants (CNAs) to ensure quality care delivery.
Emotional Support:
Provide emotional support to residents and their families. They help address concerns and fears, fostering a comforting environment
Patient-Family Communication:
communicate with patients' families about their loved ones' health and care plans, ensuring that families are informed and involved in the care process
Documentation:
Keeping accurate records of each resident's progress and health status is essential for tracking changes and adjusting care plans as necessary New leadership is here and foundational changes are happening!
