Job Overview We are seeking a compassionate and skilled Licensed Practical/Vocational Nurse to provide in‑home nursing care to patients. In this role, you will support coordinated care with the healthcare team and provide high‑quality nursing care in each patient's home. Key Responsibilities Deliver skilled nursing care as ordered in the patient's plan of care. Assist in assessing physical, psychosocial, and environmental needs. Provide patient and caregiver education. Maintain continuity of care through effective communication and accurate clinical documentation. Follow infection control and safety procedures. Participate in case conferences, in‑service training, and quality improvement programs. Uphold agency policies and professional standards. Minimum Requirements Graduate of an accredited vocational/practical nursing program. Active LVN/LPN state license. Current CPR certification. Valid driver's license, reliable transportation, and auto liability insurance. Strong interpersonal, communication, and nursing skills. Minimum 1 year of clinical nursing experience; home health preferred. Work Environment Care provided in patient homes under varying conditions. Exposure to infectious diseases, blood, and bodily fluids possible. Requires local travel and ability to work a flexible schedule. May involve prolonged standing, walking, and lifting up to 50 lbs.

Pay:

$28.00 - $35.00 per hour
