Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality, patient-centered care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will help ensure patients receive safe, effective, and personalized nursing services that promote their well-being and comfort. Duties Administer medications and injections accurately following physician orders and established protocols Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation Assist with diagnostic evaluations such as specimen collection, lab testing, and medical imaging procedures Manage IV infusions, catheter care, wound dressings, and other clinical procedures with precision and aseptic technique Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or memory care requirements Collaborate with interdisciplinary teams to develop and implement individualized care plans and discharge planning strategies Educate patients and their families on health management, medication administration, and post-care instructions Maintain detailed medical records using electronic health record (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA regulations Respond promptly to urgent situations including acute pain management, airway management, or emergency triage in various healthcare settings Requirements Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent credential Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals (Level I Trauma Center or ICU experience preferred), or outpatient clinics Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and nursing best practices Experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly desirable Demonstrated ability to perform clinical procedures including venipuncture, catheterization, sterile processing, and first aid techniques Critical care experience or familiarity with acute care settings such as emergency room or ICU is advantageous Excellent communication skills for caregiver interaction and patient education across diverse populations including geriatric patients and individuals with disabilities Ability to work independently while collaborating effectively within multidisciplinary teams in fast-paced environments Knowledge of infection control protocols, HIPAA confidentiality policies, and long-term care regulations Join us to make a meaningful difference in the lives of our patients by delivering expert nursing care in a supportive environment that values your skills and dedication!
