Responsibilities:
Provide direct patient care to residents, including administration of medications, wound care, and tracheostomy management Conduct vital signs, monitor for changes in condition, and report observations to nursing staff Develop and implement individualized care plans in collaboration with the interdisciplinary team Participate in care conferences, rounds, and quality improvement initiatives to ensure seamless coordination of care Maintain accurate and detailed records of patient care and medication administration Collaborate with other healthcare professionals to ensure patient needs are met, and positive outcomes are achieved About the
