Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care in our skilled and long term care facility . Your expertise in nursing practices, patient assessment, and medical documentation will ensure our patients receive safe, effective, and personalized care. Duties Deliver comprehensive patient care by performing physical examinations, vital signs monitoring, and health assessments. Administer medications and injections accurately following physician orders and medication management. Manage patient intake and discharge processes, ensuring proper documentation in electronic health records (EHR) systems. (Point Click Care) Provide specialized care for patients with complex conditions such as dementia, Alzheimer's disease, or those requiring hospice or palliative care. Support emergency situations with airway management, acute pain management, and basic life support (BLS) techniques. Maintain strict adherence to HIPAA regulations and infection control protocols to safeguard patient confidentiality and safety. Skills Proven experience working with diverse populations including geriatrics, and individuals with developmental disabilities. Strong knowledge of physiology, anatomy, ICD coding (ICD-9/ICD-10), and medical terminology relevant to acute care environments. Skilled in performing catheterization (including Foley catheters), phlebotomy, and sterile technique procedures. Ability to assess patient conditions effectively through physical examinations and diagnostic evaluation techniques. Excellent communication skills for triage assessments, patient education (including health coaching), and case management activities. Knowledge of medication administration protocols including injections.
