Job Overview We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) - Staff Nurse to join our dynamic healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings. In this role, you will deliver essential nursing services, support patient assessments, and ensure the highest standards of safety and confidentiality in accordance with healthcare regulations. Responsibilities Provide direct patient care through assessments, vital sign monitoring, and medication administration while adhering to established protocols. Document patient information accurately using Electronic Health Record (EHR) systems and ensure compliance with HIPAA regulations. Support care coordination efforts including discharge planning, case management, and client communication. Perform diagnostic evaluations such as specimen collection and phlebotomy. Educate patients and caregivers on health management strategies including medication use, nutrition, and chronic disease management. Requirements Valid licensure as a Licensed Practical Nurse (LPN) from an accredited licensing board. Proven experience in diverse clinical environments such as long-term care facilities, hospitals, or outpatient clinics. Knowledge of EMR/EHR systems. Familiarity with medical terminology, anatomy knowledge, and vital signs assessment procedures. Strong communication skills to effectively collaborate with multidisciplinary teams and provide compassionate patient care.
