Job Description
LPN, Palliative Care Stillwater Medical at Home needs a Palliative Care LPN to join our team! The Licensed Practical Nurse in this position will act as Care Coordinator and be responsible for organizing the day-to-day operations for the Palliative Care program. As an experienced nurse, he or she will participate in identifying patients appropriate for Palliative Care, be able to determine when additional resources are needed and prepare patients and families for transitioning to other levels of care in our home care continuum. The professional nurse will build relationships with community partners, collaborate with Inpatient, Home Health and Hospice in order to plan and direct services to meet the needs of individuals and families in the community. The LPN plans, organizes and directs palliative care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. The registered nurse will act as Case Manager/Care Coordinator for palliative care patients.QUALIFICATIONS
Licensed Practical nurse with current licensure to practice in the state of Oklahoma. Maintains a current BLS certification as required by organizational policy. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. Minimum of two years experience, at least one of which is in the area of public health, home care, palliative care or hospice nursing; has training in palliative care and/or hospice care Self-directed and able to work with minimal supervision. Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist. Shows ability to organize and prioritize workload independently.PHYSICAL REQUIREMENTS
Must have adequate perception of sounds or adequate hearing with corrections. Adequate vision, or correctable with glasses/contacts, to prepare reports and read written materials. Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment.
