Position Summary:
We are seeking an experienced LVN/LPN - Rapid Discharge Unit (Medical-Surgical) to provide safe, high-quality nursing care to patients preparing for discharge from an acute care hospital. The ideal candidate will collaborate with RNs, physicians, case managers, and the multidisciplinary team to ensure patients are medically stable and discharge-ready. Responsibilities include conducting patient assessments, administering medications, maintaining accurate documentation, reinforcing discharge instructions, and educating patients on post-discharge care. Candidates must have at least 2 years of recent acute care experience , strong clinical judgment, excellent communication skills, and the ability to thrive in a fast-paced Medical-Surgical environment while supporting safe and efficient patient transitions.
Roles & Responsibilities:
Perform patient assessments, monitor vital signs, administer medications, and provide nursing care to patients preparing for discharge from the Medical-Surgical unit. Collaborate with RNs, physicians, case managers, and the multidisciplinary team to ensure patients are medically stable and discharge-ready. Complete accurate and timely documentation in the electronic medical record (EMR) while maintaining compliance with hospital policies and regulatory standards. Reinforce discharge instructions, educate patients and families on medications, follow-up care, and post-discharge treatment plans. Coordinate discharge activities, identify potential barriers to discharge, and promote safe, efficient patient transitions from the hospital to the next level of care.
Skills:
Strong knowledge of acute care Medical-Surgical nursing practices and patient discharge processes. Proficient in patient assessment, medication administration, vital sign monitoring, and accurate EMR documentation. Excellent communication and patient education skills with the ability to collaborate effectively in a multidisciplinary team. Strong critical thinking, clinical judgment, and problem-solving abilities in a fast-paced acute care environment. Ability to prioritize tasks, manage multiple patients, and ensure safe, timely, and efficient patient discharges.
