In this vital role, you will provide high-quality nursing care to patients in their homes, promoting health, safety, and independence. Your expertise will help deliver personalized care plans, monitor patient progress, and collaborate with families and healthcare teams to ensure optimal outcomes. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring skilled nursing support in a community setting. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation Administer medications and injections accurately following physician orders, including IV infusions and other advanced therapies Manage wound care, catheterization, and sterile procedures with precision and adherence to infection control protocols Monitor patients' conditions closely, recognize signs of deterioration, and triage urgent needs effectively Document all patient interactions thoroughly using electronic health record (EHR) systems such as Epic or Cerner Educate patients and caregivers on health management, medication administration, and safety practices Coordinate discharge planning and collaborate with interdisciplinary teams to ensure seamless continuity of care Experience Valid state licensure as a Licensed Practical Nurse (LPN) with current certification Prior experience working in home health care or community-based nursing environments preferred Demonstrated proficiency with EMR/EHR systems like Epic or Athenahealth for accurate documentation Experience with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities, dementia care, and hospice patients Knowledge of complex medical procedures such as IV insertion, tube feeding management, ventilator support, and PICU or ICU experience highly desirable Familiarity with long-term care regulations, HIPAA compliance, and medical documentation standards Strong communication skills for caregiver education, client interaction, and interdisciplinary collaboration Join us to provide compassionate nursing care that empowers individuals to live healthier lives at home.
