As an LPN, you will be responsible for providing direct patient care to residents, ensuring their comfort, safety, and well-being, and working alongside our healthcare team to deliver the highest quality of care. This is an excellent opportunity for a skilled LPN to work in a collaborative and supportive environment with a focus on patient-centered care. Responsibilities Provide direct nursing care to residents, including administering medications, treatments, and daily care routines as prescribed by physicians. Monitor residents' physical and emotional condition and report any changes to the supervising RN or physician. Assist with the development and implementation of individualized care plans, ensuring they reflect residents' needs, preferences, and health goals. Perform routine assessments, including vital signs, and document changes in residents' conditions. Coordinate and communicate with other healthcare professionals, family members, and staff to ensure the continuity of care. Assist with resident activities of daily living (ADLs), including bathing, dressing, feeding, and mobility. Ensure the safe and accurate administration of medications, including tracking, documenting, and reporting as needed. Maintain accurate and up-to-date resident records in compliance with facility policies and state regulations. Respond promptly to emergencies, providing first aid and other interventions as necessary. Ensure a clean, safe, and respectful environment for residents, promoting their dignity and independence. Requirements Current, unencumbered LPN license in the state of Oregon. Prior experience in long-term care, skilled nursing, or similar healthcare setting preferred, but not required. Proficiency in venipuncture, IV infusion, and aseptic techniques. Familiarity with medical documentation standards and procedures. Ability to work effectively in a fast-paced environment Strong interpersonal skills to communicate effectively with patients, families, and healthcare team members.
