What You'll Do:
Provides direct patient care. Implements plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s). Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Director of Clinical Services. Uses equipment and supplies effectively and efficiently. Participates in personal and professional growth and development. As a Hospice LPN at Signature , you'll be a beacon of compassion, a champion of dignity, and a superhero of personalized care! Here's what we're looking for: Licensed Practical Nurse (LPN) licensure in Oregon 1+ Year of Nursing Experience Ability to shine solo or rock it out as part of a dynamic team Balancing clinical excellence with an abundance of empathy and zest for life Hold a current BLS certification from AHA Cruise around town with your own reliable vehicle But wait, there's even MORE to love!
