Home Care / Field-Based Nursing Position Overview We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) with a strong background in Home Care to join our healthcare team. The ideal candidate will have at least 2+ years of home health experience and must be highly proficient in wound care management . This role involves providing skilled nursing care to patients in their homes while ensuring high-quality clinical support, patient safety, and continuity of care. Candidates must be comfortable traveling within the assigned coverage areas and working independently in a home care environment. Key Responsibilities Provide skilled nursing care to patients in home care settings Perform and assist with wound care treatments, dressing changes, and skin assessments Monitor patient conditions and report changes to the supervising RN or physician Administer medications and treatments as prescribed Assess patient needs and support individualized care plans Document patient care accurately in EMR systems Educate patients and family members on treatment plans and home care instructions Maintain infection control and patient safety standards Coordinate care with interdisciplinary healthcare teams Travel between assigned patient homes within designated zip codes Requirements Active and unencumbered LPN License Minimum 2+ years of Home Care experience required Must be proficient in Wound Care to be considered Strong clinical assessment and patient care skills Excellent communication and documentation abilities Reliable transportation required for patient visits Ability to work independently in field-based settings Preferred Qualifications Previous experience in skilled home health or community nursing Experience with EMR documentation systems Strong time management and organizational skills Compassionate and patient-focused approach to care Why TalentBurst?
